The Care Manager RN is responsible for managing patients with chronic illnesses by coordinating care among multiple providers to improve health outcomes. This role involves patient education, skilled assessment, and collaboration with healthcare and community resources to support self-management and quality of life. The RN works under minimal supervision and may perform additional ambulatory care duties within established nursing standards.
Description
The Care Manager RN is responsible for managing patient populations with chronic illness and/or multiple comorbidities. This individual works in coordination with multiple PCPs and serves as the central link to an identified population of patients. Through skilled assessment, education, collaboration and coordination of healthcare and community resources, the RN Care Manager Navigator assists patients to gain self-efficacy/management skills, achieve optimum functional health status, and quality of life. Assists patients/families, staff and systems to achieve high quality, evidence based, cost-effective, and patient focused outcomes. This person works under minimal supervision within the general policies and standard practices and the nursing code of ethics, including Chapter 18.88 RCW. May perform other ambulatory care RN duties as required.
